I will get to most portal account requests within a day or two of submission and will need your first and last name, email address, and social security number to create the account. Once it's created, you will receive an activation email with a link to create a password. There are also links in the activation email to download the smartphone app that goes with the portal, so you can use your phone to upload photos of your documents. Just ensure Live is turned off when capturing your forms, and that each photo is clear and fully in frame.
